概念:

1.流行病学任务的三个阶段:揭示现象,找出原因,提供措施。

“揭示现象”时,开展描述性工作,即通过描述性流行病学方法来实现。

“找出原因”,借助分析性流行病学方法来检验或验证所提出的病因假说。

“提供措施”,并进一步确证措施的有效性,这要用实验流行病学的工作来完成。 2.流行病学的三种基本方法:流行病学则以观察法、实验法和数理法为其基本,其中尤以观察法为最重要。

3. 描述流行病学主要是描述疾病或健康状态的分布,起到揭示现象、为病因研究提供线索的作用,即提出假设。而分析流行病学主要是检验或验证科研的假设。实验流行病学则用于证实或确证假设。

4.流行病学特征:群体特征,对比特征(核心),概率论和树立统计的特征,社会医学的特征,预防为主的特征,发展的特征

5.比例有两类,一、反映事物静止状态内部构成成分占全体的比重,也称构成比例,二、与动态的发生变化概率密切相关的发生频率比例,它反映一定时间内,发生某种变化者占全体的比例。

6发病指标:发病率,患病率,罹患率,续发率,感染率

7.某地某病发病率和该病的病程在长时间内保持稳定时,患病率=发病率?病程,即:P=ID 。患病率通常用来表示病程较长的慢性病的发生或流行情况。

8.感染率主要用于隐形感染 9.死亡指标:死亡率,病死率,生存率,累计死亡率 10.疾病年龄分布的分析方法:横断面分析(不能说明不同年代出生的各年龄组死亡趋势 ),出生队列分析。 11.抽样调查选择研究对象的基本原则是保证每一个研究对象是以等同的机率从其总体中选出的,即研究样本要有代表性。

12.病例对照研究选择对照的原则是代表性,可比性,暴露不能作为选择对照的依据。

13. OR 的含义与相对危险度相同,指暴露组的疾病危险性为非暴露组的多少倍。疾病率小于5%时,OR 是RR 的极好近似值。无论以暴露比值和非暴露比值计算,或是以有病比值和无病比值计算,比值比的结果都是一样的,OR 恒等于ad/bc 。

14.选择偏倚:选入者与未选入者某些特征上存在差异引起入院偏倚、现患-新发病例偏倚、检出征候偏倚、时间效应偏倚等

15.队列研究的对照选择基本要求是尽可能与暴露组的可比性,即对照人群除未暴露于所研究的因素外,其它各种影响因素或人群特征

(年龄、性别、民族、职业、文化程度等)都应

尽可能地与暴露组相同。

16.研究中现患率,暴露率,发病率越接近50%,样本含量就越大。

17.RR 具有病因学的意义,AR 更具有疾病预防和公共卫生学上的意义

18.失访偏倚是选择偏倚中的一种。

19.实验性研究的注意事项:研究对象的排除,实验对象的退出,预实验 20.诊断试验中的样本含量:①待评价筛检试验的灵敏度,②待评价筛检试验的特异度,③显著性检验水平α,④容许误差δ。当灵敏度和特异度均接近50%时,用灵敏度估计病例组所需样本量,特异度估计对照组所需样本量。

24.阳性预测值与灵敏度成正比;阴性预测值与特异度成正比; 筛检试验的灵敏度越高,阴性预测值越高;筛检试验的特异度越高,阳性预测值越高。特异度的改变对于阳性预测值的影响比灵敏度改变更大。受试人群的患病率越低,阳性预测值越低,阴性预测值越高。
