社区老年慢性病健康管理模式研究

[摘要] 目前老年慢性病的发生率逐年升高，对其预防和治疗也经历了一个发展的过程，社区作为老年人活动的基本场所，在慢性病防治中发挥着基础性的作用，该文通过当前我国社区老年慢性病健康管理中存在的一些问题进行了分析，针对健康管理的特点，提出相应的解决方案，以研究符合我国的社区老年慢性病健康管理模式。

[Abstract] At present， the incidence rate of elderly chronic disease gradually increases， and the prevention and treatment of it has experienced a development process. As a basic place for the activities of the elderly， community plays a basic role in the prevention and cure of chronic disease. The paper analyzes some problems existing in the health management of community elderly chronic disease of our country at present and provides corresponding solutions aiming at the health management characteristics in order to research the health management mode meeting the community elderly chronic disease of our country.

随着我国改革开放的逐渐深入和经济社会的不断发展，逐渐步入老龄化社会，老年人口所占人口比重逐年上升。因此，老年慢性疾病已经成为我国疾病的主要类型和致死的首要原因，日益严重威胁人类的健康。根据世界卫生组织的预测，到2020年慢性疾病造成的死亡将达到我国死因的70%[1]。社区卫生服务的主要人群为老年人，重点是患有慢性病的老年人。该文
对某城市社区的老年人慢性病健康管理的模式进行了分析研究，为提高老年慢性病患者的生存质量提供一点建议。
1 当前我国社区老年慢性病健康管理中存在的一些问题
目前慢性疾病的预防理念是提倡三级预防，最重要的是以预防为主，通过宣传、实行“合理膳食、适量运动、戒烟戒酒、心理平衡”的科学生活方式，从而达到减少疾病发生的目的[2]。老年人的主要生活地点是社区，由于目前我国社区发展迅速，但是社区配套的卫生服务
却相对短缺，并且社区卫生服务站的服务水平和服务理念存在较多不足。因此其对老年慢性病的管理和控制作用不大。主要有以下几点问题。
